What is a transoesophageal echocardiogram? A Consultant Cardiologist's guide
A transoesophageal echocardiogram, often shortened to TOE, is a detailed ultrasound of the heart taken from just behind it. A thin, flexible probe is passed gently into the oesophagus, which lies directly behind the heart, giving much clearer views of the heart valves and the upper chambers that are possible from the chest wall. It uses sound waves only, with no radiation.
Because the probe sits so close to the heart, a TOE can show fine detail that a standard scan through the chest may miss. This makes it particularly valuable for assessing valve disease precisely, for looking for blood clots in the upper chambers, and for examining the heart before certain procedures. At the Harley Street Main Hospital, 35 Weymouth Street, London W1G 8BJ, the study is performed and interpreted by Professor Michael Henein, a Professor of Cardiology whose cardiac imaging expertise is recognised internationally. A transoesophageal echocardiogram demands considerable skill to perform and interpret, and here it is carried out by an experienced consultant throughout.
Who needs a transoesophageal echocardiogram?
A TOE is often recommended when a standard echocardiogram cannot show the valves clearly enough, when detailed assessment of a leaking or narrowed valve is needed, when a source of a stroke is being sought, or before certain heart procedures. It is also used to guide decisions about valve treatment. Patients from London, other UK cities and abroad.
What to expect on the day
1
You are asked not to eat or drink for 4 hours before the procedure, and you should arrange for someone to accompany you home.
2
The back of your throat is numbed, and light sedation is usually given to keep you comfortable.
3
4
You rest until the sedation wears off, and Professor Henein discusses the findings with you afterwards.
Types of assessment offered
A transoesophageal echocardiogram is often used to add detail after a transthoracic echocardiogram, and it may be combined with other tests where a fuller picture is required. Where the question concerns the coronary arteries rather than the valves, cardiac CT may be more appropriate. The plan is tailored to your condition, so that this more detailed study is used only where it genuinely adds value.
Why choose Professor Henein for a transoesophageal echocardiogram
A TOE is a more specialised study that relies on the experience of the clinician performing it, both for comfort and for accurate interpretation. Professor Henein is a Consultant Cardiologist and Professor of Cardiology with internationally recognised clinical and imaging expertise, including fifteen years at Royal Brompton Hospital, and a significant academic record.
Because he manages the complete patient journey from diagnosis through treatment, the findings feed directly into your ongoing care rather than a further referral.
Common questions about a transoesophageal echocardiogram
Why is a transoesophageal echocardiogram needed?
It gives much clearer views of the heart valves and upper chambers than a standard scan, which is valuable for detailed valve assessment and for finding blood clots.
Is the test uncomfortable?
The throat is numbed and light sedation is usually given, so most people are comfortable during the scan.
How should I prepare?
You are asked not to eat or drink for 4 hours beforehand, and to arrange for someone to accompany you home because of the sedation.
How long does it take?
The scan usually takes around 20 minutes, with additional time to recover from the sedation.
Can I drive home afterwards?
No. Because light sedation is usually used, you should arrange for someone to accompany you home and should not drive for the rest of the day.
